Job Description:
The Connect Care- Behavioral Health Clinical Social Worker utilizes clinical expertise to perform psychosocial assessments, implement plans of care with the appropriate care team, assess crisis situations to appropriately provide needed resources and or referrals for the patient, and to provide individual treatment as indicated. This position works collaboratively with patients, their family and/or caregivers, and providers so they can self-manage their care, optimize their functional health status, have quality outcomes, and use the health system appropriately. Develop, document, and implement plans which will provide the appropriate resources to address social, physical, mental, emotional, spiritual, and supportive needs. Position Details:
Clinic hours are 7a-7p, 7 days per week. Closed holidays. Flexible schedule, 40 hrs. per week.
This role will be a part of the Connect Care- Behavioral Health Team.
Looking for a candidate with experience in the treatment of adolescents and/or pain management.
Will accept LCSW or LMFT licensure.
Salary will be based on licensure and experience.
